Question Types
Text
Allow the respondent to enter free-text as a response.
Capture an email address.
URL
Capture valid URLs.
Yes/No
Ask your audience to answer either yes or no.
Color Picker
Select the desired color with a color picker, find colors by hex code, or restrict colors to specified swatch.
Number
Capture a numerical response (whole or decimal).
Phone Number
Capture a phone number, optionally in a specific format.
Address
Capture a physical address
Country
Choose from a list of all countries.
Appointment
Schedule appointments and optionally connect to your Google Calendar to automatically handle availability and create events.
Date
Type or select a valid date.
Time
Type an exact time.
Scale
Choose on a scale of X to X.
Slider
Select a value from a minimum and maximum scale
Multiple Choice
Choose one or more of your options - text, and/or images! This can be configured to behave like a checkbox (multiple answers) or a radio button (only one answer).
Dropdown
Choose option(s) from dropdown.
Image Upload
Upload one or more images.
File Upload
Upload one or more files.
Signature
Draw a signature that is stored as an image.
Price
Add a simple price to the form, or allow the respondent to enter in their own price (great for donations).
Product
Select from a list of products, with SKU and quantity management built-in.
Subscription
Choose from a list of Stripe or Braintree subscription plans for the submitter to subscribe to.
Calculation
Get creative with Excel-style functions that are run live on respondents' answers. Great for complex calculations, or transforming answers into more desirable formats.
Hidden
Used to pre-fill answers from the URL and not show them to the submitter.
Rank
Show a list of pre-determined options in a list, and ask respondents to rank them in order.
Rating
Show up to ten symbols of your choosing, and allow respondents to select a rating using these symbols.
Matrix
Show a grid with a list of questions along each axis, and allow respondents to select one or more options on the grid per row.
