See all integrationsTo create a Booking Form with Paperform, simply choose a suitable template from the library, connect it to your Google Calendar, and start customizing it with your questions and design. You can add, remove and edit the existing fields to collect the information you need.
Once you’re happy with your form, embed it on your website so your customers can book an appointment with you directly from your website.
The information that should be on a booking form depends on what type of booking you’re collecting information for. Some common fields to include are customer’s contact details (name, email, phone number), date and time of booking, and payment information.
Make sure your form is easy to navigate and as short as possible. Ask only the necessary questions, and offer multiple payment options.
Select the Booking Form Template you want to use, then head over to the Editor, and click on your Appointment question.
Click on ‘+ Calendar’ to connect to a new Google Calendar, then select the calendar you want to use for your appointments.
Next, set the event's details, including the Title, Description, and Location. These details will be saved to your calendar when the appointment is created and will also be used to display that appointment in your submission.
Once you have connected your Paperform Appointment Form with your calendar, any appointments that are submitted through the form will be automatically added to your calendar.

After customizing the form to your liking, click the "Publish" button in the top-right corner of the editor. Then, select the "Embed" option. Choose the type of embed you want to use, such as "Inline" or "Popup", and then copy the embed code.
Go to your website and open the page where you want to add the form, then paste the embed code into the HTML code of the page where you want the form to appear.
Save and publish your website.
