An Appointment Form is a type of form that is used to collect information from customers or clients who want to book an appointment with a business or organization. The information that is collected on the form can vary depending on the business or organization, but it typically includes the customer's name, contact information, and the date and time of the appointment.

1. Select the Appointment Form Template you want to use, then head over to the Editor, and click on your Appointment question.
2. Click on ‘+ Calendar’ to connect to a new Google Calendar, then select the calendar you want to use for your appointments.
3. Next, set the event's details, including the Title, Description, and Location. These details will be saved to your calendar when the appointment is created and will also be used to display that appointment in your submission. Once you have connected your Paperform Appointment Form with your calendar, any appointments that are submitted through the form will be automatically added to your calendar.
