Use this maintenance request form template to allow your customers easily contact you. We created this maintenance form for an imaginary company called Brody Bros Co..
The form template is quite lengthy and detailed. Once you import the form into your Paperform account, you might start customizing it from the top. It's a good idea to add your company's logo and optionally a photo of your team or the equipment that you're selling.
Then, start tweaking the question fields. In Paperform's editor, you can choose among more than 20 different question fields. It's always a good idea to use specialized fields to collect information like dates, addresses, and phone numbers (to ensure that all the information will be properly formatted). There's also a special Signature field if you want to collect formal signatures from your customers.
Paperform also allows you to schedule maintenance appointments through your forms. To get started, add an Appointment question field to your form. Some defaults are already set for you, but you can always edit those options in the Configure (the gear icon) menu. Consider integrating your Google calendar to ensure you aren't double-booked and automatically add new appointments to that calendar.
Finally, set your form live. You can share your maintenance request form with your customers either via a direct link or embed it on your website.
