This template represents a fashion brand, but you can easily make that form yours by heading over to the theme settings. Change colors, fonts, add your company's logo and relevant images — the better your responders can relate to your brand, the more likely they are to fill out the survey.

You can use a variety of form fields to collect feedback in the best format possible. If your survey has quite a few questions, it might be a good idea to break down your form into multiple pages. A navigation bar will appear below once you have two or more pages. It's a good idea to show the responders what percentage of questions has been completed — it'll psychologically encourage them to continue.

You can export your results in the preferable format, or integrate your form with Google Sheets and synchronize data automatically.
