Consultation Form Template

We created this form template to help you collect client consultation requests. To start editing it, click the button "Use this template" below the description.

You must have noticed that this form asks one question at a time. You can easily change this by removing the page breaks. Be careful not to make your form too long and complicated to fill out, though — this might significantly impact your completion-rates.

To make sure that you don't get any clashes in your schedule, you might connect the Google Calendar to your form and specify your availability. You can also tweak the length of meetings booked through the consultation form (e.g., 15 minutes, 30 minutes, 60 minutes).
