Create a beautiful online registration form for your club with this Paperform template. This particular template was created for a school holiday club — but you can easily customize this form and make it truly represent your organization. Click the button "Use this template" below to get started.
You might have noticed that there are no form-fields on the first page. If your form is quite lengthy, it might be a good idea to use that first page to introduce your organization and thus build trust with your responders.
The second page of the registration form contains several questions, all of which are set up to be required (your responders can't navigate to the next page or submit the form unless all the required questions are answered). To change whether a question required or not, go to ‘Configure’ to the right of the question field.
There's also a conditional block on that page, which appears only if the question Does the Student have any allergies, ailments, or restrictions? is answered affirmatively. You can configure the question logic in the Configure menu of each field.
