Shift Change Form Template

If your employees work in shifts, you need to set up a simple yet reliable system for them to request shift changes when required. Using an online form for this purpose might be an elegant solution.

The form template has two signature fields: one for the employee that is requesting a shift change and another for the person that would be covering them. If you have a tablet or a laptop installed in your office, both employees could easily fill out the same form and leave their electronic signatures. However, if the form is being filled out individually, you might only collect one signature (from the employee that is requesting a shift change).
