Prayer Request Form Template

If you're working for a spiritual organization, you might be looking to create a prayer request form. We created this template to help you get started faster.

The form template below is very simple. The first three fields collect basic information about the prayer request. The last field allows your responders to make an optional donation. If they input a numerical value, upon the submission, they'll be directed to the payment gateway of your choice (Stripe, Square, PayPal Business, Braintree) to complete the donation.
