Employee Resignation Form Template

If your company's employee turnover is naturally high (you might, for example, be employing seasonal workers), this online form might help you to streamline the process of submitting and processing resignations.

The form template by itself is quite simple. Your employees might fill out their names, employee ID numbers (if they have any), and select on which day they'd like to resign. They might also optionally attach a signed resignation letter if that's required by law. Otherwise, you might also collect their electronic signatures through the form.

In the After Submission settings menu, you might set up automated emails to be sent out to the company's manager or the human resources department once the form is submitted.
