Signature Form Template

Use Paperform to collect electronic signatures online. We created this template to showcase how your signature form might look. To start editing it, click the button "Use this template" below the description.

Paperform supports the capture of signatures on Pro and Agency plans. You can add a new signature form by clicking anywhere on your form, inserting a new question, and changing its type to Signature. The field will require the responder to sign either by drawing with a touch screen or with the curser, depending on their device.

Once the signature form lands in your Paperform account, replace the Lorem Ipsum with your own text or form fields. You can easily insert images or embed videos inside of the forms to make them more interactive.
