Volunteer Registration Form Template

We broke down this volunteer registration form into four pages. The first page introduces the volunteering program and contains contact information of an organization. The second page collects all the necessary personal information of the person who's applying to volunteer. The following page covers the logistics, asking on which days they're available and what volunteering role they're interested in. Finally, the last page contains some more information for the responders and a submit button. While editing your form, you can easily add additional questions, tweak the existing ones, or change the number of pages (hint: use page-breaks).
