Matrix Field Form Template

Matrix questions sound complicated, but they're simple. Essentially, matrix form fields are just multiple questions presented on a grid.

The matrix field works the same as any other field in Paperform. Select the question type from the menu, or type /matrix slash command in Paperform's editor. From there, click the Cogwheel icon to go into the configuration menu and set up your question.
