Attendance Form Template

You might have noticed that this form template is quite simple. It collects the names of your guests, their phone numbers, and asks whether they're bringing anyone with them. While editing your form, you can easily add additional fields. You can also insert text blocks and embed images or video (might come in handy to e.g. inform your guests of the event's dress code).
