This feature is available on Agency plans.

If you have an Agency account, you can choose the image you want to use for your form icon. Form icons are displayed anywhere that identifies your form's webpage. This includes tabs, bookmarks, toolbar apps, history results, and search bars.

To add an icon to your form:

  1. Go to Configure → Details.
  2. Click "CHOOSE A FILE" below "Icon."
  3. Drag and drop an image from your computer, or click to select an image from a folder on your computer.

  • The image file you upload should be at least 512 x 512 pixels.
